Immunization Information for Incoming 6th Graders

Due to the recent legislation regarding immunization requirements for incoming 6th grade students, all students born on/or before September 5, 1996 and entering 6th grade in Sept 2007 will be required to:

• show proof of immunization and/or immunity for Varivax (Chicken Pox)

• receive a booster dose of Tdap (Diptheria, Tetanus, Pertussis) before the first day of school.

All 6th grade students born after September 5, 1996 will be required to provide proof of immunization and/or immunity before the last day of his/her birthday month in order to remain in school.
